The staff at RMIA draws on a long commitment to
excellence in the field of reproductive endocrinology and,
more specifically, infertility. Over the last twenty years, advances and innovations in the treatment of infertility have been made, resulting in the increased availability of In Vitro Fertilization (IVF). Unfortunately, the refusal of insurance companies to cover this treatment option has prevented infertile couples from receiving its benefits.

In an attempt to resolve this paradox, the staff at RMIA pioneered and championed the concept of a financial risk-sharing between patient and fertility center. In 1995, the Fertility Cost Warranty Program (FCWP) was introduced in an effort to make IVF more affordable to our patients. We are pleased to announce that the American Society of Reproductive Medicine (ASRM) has endorsed this risk-sharing warranty program, and it is now accepted as a legitimate treatment alternative provided that patients receive full disclosure and that treatment guidelines are observed.

The Fertility Cost Warranty Programs offer qualified patients up to three fresh cycles of IVF for a predetermined cost.

  • Refundable portion is due back if the patients do not achieve a live birth plus 30 days of life.
  • Program cost is locked in at the time of the patients prescreening orders, which will be made by the RMIA physician. Patients will need to complete their prescreening within 60 days.

Age < 39 years of age (at time of consent signing)       
Body Mass Index (BMI)
≤ 35.0
Follicle-Stimulating Hormone (FSH) ≤ 10 or normal ovarian response
Sperm

≥  500,000 motile (If less, may qualify for the
Low Sperm Amendment. Call Business Office for options.)

Uterine Cavity Normal
Previous Unsuccessful IVF cycles Approval from RMIA physician
Miscarriages > 2 miscarriages requires approval from RMIA physician.
Additional testing may be needed.

Corrective surgery for:

  1. Distal tubal occlusion
  2. Tubal disease associated with
    ≥ 1 tubal pregnancy
  3. ≥ 2 tubal pregnancies, irrespective
  4. of tubal status

  1. Smoking (female and male)
