HOPE Program

The HOPE program was created in 2007
to build on the success of the Fertility Cost
Warranty Program (FCWP) by including more
services into the HOPE program.

Additional Requirements
(in addition to the FCWP criteria)
 
Age Less than 35 years old
Body Mass Index (BMI)
> 19 and < 33.0
Sperm ≥  500,000 motile
Previous Unsuccessful IVF cycles None

Cost

RMIA will ACCEPT INSURANCE ONLY for the following services performed at RMIA:

  • Program Start
    • Sonohysterogram
    • Trial Transfer
    • Informed Consent Signing
  • Injection Teaching
  • Serum Pregnancy Tests
  • Confirmation of Pregnancy Ultrasounds
  • Hyperstimulation checks and related services

The following services are INCLUDED Refundable Fee Amount:

  • Ovarian Stimulation Medications
    • Bravelle (FSH) or equivalent
    • Menopur (HMG) or equivalent
  • IVF cycle monitoring (performed at RMIA or approved satellite clinic)
    • Ultrasounds (starting with baseline)
    • Blood work
    • Office visits
  • Aspiration of oocytes (including anesthesia services) performed at RMIA
  • Fertilization process utilized by the Embryology Laboratory to include:
    • Intracytoplasmic Sperm Injection (ICSI)
    • Assisted Hatching
    • Extended Culture
  • Cryopreservation of embryos
  • Storage of embryos – one year from the date of initial cryopreservation
  • Fresh and corresponding frozen (if applicable) embryo transfers
