Fertility Cost Warranty Program: Program Cost

Reproductive Medicine and Infertility Associates (RMIA) offers the following Fertility Cost Warranty Program options:

  1. FCWP in which the patient uses her own eggs. (See also our Fee for Service Program)
  2. FCWP in which the patient uses donor eggs. (See also our Fee for Service Program)

Fertility Cost Warranty Program with patient's own eggs:

Female partner <35* (meets HOPE criteria) $17,000 100% refundable
Female partner <35* $12,500 refundable/$3,500 non-refundable
Female partner ages 35-36 $15,000 refundable/$4,000 non-refundable
Female partner ages 37-38 $17,500 refundable/$4,500 non-refundable
* Single Embryo Transfer (SET), add 10%

Fertility Cost Warranty Program using Donor Eggs:

Cost of the program is not affected by the age of the female partner. There are two options for the FCWP with donor eggs: one donor assigned to one recipient $36,000 with $26,000 refundable/$10,000 non-refundable; the second option is FCWP with donor eggs where the egg donor is shared between co-recipients, $24,000 100% refundable.

  • Retrieval
  • Fertilization
  • ICSI
  • Assisted Hatching
  • Transfer
  • Cryoperservation storage of cryopreserved embryos at RMIA for up to 12 months after retrieval

Excluded

  • Prescreening
  • Medication
  • Monitoring (for those patients who do not have insurance coverage for their monitoring, RMIA offers an option similar to the FCWP for the duration of the program for an additional 25% of the total cost of the program [BASE COST plus Low Sperm Amendment and/or SET option if applicable])
  • Non-RMIA consultations
  • Hospitalization/complications
  • Male services/facility costs
  • Contracted services
  • Embryo storage beyond the first 12 months
  • Pregnancy testing
