Eligibility:

Female <51 years old (patients aged 45-50 must have clearance from a perinatologist prior to pre-screening. No exceptions.)

• At the time of embryo transfer
• Normal uterine cavity
Unknown Donor
1. Services billable to insurance
• Prescreening (recipient only)
• Monitoring of recipient
• Medications of recipient
2. Services usually not covered by insurance
• Donor associated costs - $4,000
• Donor medications - $3,000 (est)
• Monitoring of Donor - $1,500 (est)
• Regular IVF cycle - $7,500
(includes egg retrieval, fertilization and fresh embryo transfer to recipient)
Known Donor
1. Services billable to insurance
• Prescreening (recipient only)
• Monitoring of recipient
• Medications of recipient
2. Services usually not covered by insurance
• Prescreening of Known Donor
• Donor medications - $3,000 (est)
• Monitoring of Donor - $1,500 (est)
• Regular IVF cycle - $7,500
(includes egg retrieval, fertilization and fresh embryo transfer to recipient)
 
Additional Options
ICSI
   $1,200.
Assisted Hatching
   $400.
Freezing of embryos
   $1,000.
Storage of frozen embryos
   $60/month.
