Eligibility:
 • Female egg donor <39 years old at the time of egg aspiration and with sufficient ovarian reserve
• Gestational carrier <45 years old at time of transfer and with normal uterine cavity
1. Services billable to insurance
• Prescreening (couple and carrier) $3,500 (est)
• Medicating fresh cycle (patient and carrier) $3,500 (est)
• Fresh cycle monitoring (patient and carrier) $1,800 (est)
2. Services usually not covered by insurance: IVF base price $7,500 includes
• Egg retrieval costs
• Fertilization costs
• Fresh embryo transfer costs
