Eligibility:

Female <46 years old at time of egg aspiration if using own eggs

• Sufficient ovarian reserve
• Normal uterine cavity
1. Services billable to insurance
• Prescreening
• Medications fresh cycle
• Fresh cycle monitoring
2. Services usually not covered by insurance:
IVF base price $7,500 includes
• Egg retrieval costs
• Fertilization costs
• Fresh embryo transfer costs
Additional Options:
ICSI

Assisted Hatching
   $400
Freezing of embryos
   $1,000
Storage of frozen embryos
   $60/month
