Low Sperm Amendment

The purpose of the Low Sperm Amendment is to allow certain patients a chance to participate in the Fertility Cost Warranty Programs (FCWP) even though the male patient has less than 100,000 motile sperm in his semen, a condition which would otherwise disqualify them from participation in the programs.

Based on the severity of the male factor, patients will fall into 1 of 3 categories:

  1. Male has < 100,000 sperm (due to testes not functioning well)
  2. Male has 0 sperm (due to testicular failure)
  3. Male has 0 sperm (due to obstructive azoospermia, most commonly resulting from previous vasectomy)

Categories 1 and 2: the total cost of the warranty program will increase by 20%.

Category 3: $1,000 will be added to the total cost of the warranty program, all of which goes towards the refundable portion. 

Additional screening costs, such as a comprehensive evaluation by a urologist, as well as surgical costs may be incurred. Donor sperm may be required as a possible back-up.
