Completing forms before your visit allows us to obtain all the necessary medical and insurance information we will need so your visit can be as streamlined as possible. Beginning July 1st, 2024, credit card payments will be subject to an additional 3% processing fee to cover the cost of the payment service providers to authorize and complete credit card transactions. No additional fees apply to payments made with debit cards, personal check or electronic funds transfer (EFT). RMIA does not accept cash.
