Even with the availability of in vitro fertilization (IVF), success is nearly impossible without an appropriate sperm sample. An absence of sperm does not mean sperm cells are not produced. Understanding severe male infertility
There are various degrees of male-factor infertility, but in severe cases, the condition can make achieving pregnancy challenging. Severe male infertility is often diagnosed when sperm counts are extremely low or completely absent. Cases are further categorized into obstructive and non-obstructive azoospermia. Obstructive azoospermia occurs when sperm production is normal, but a blockage prevents sperm from reaching the ejaculate. A prior vasectomy, infection, or congenital absence of the vas deferens are typical causes of obstructive azoospermia. Non-obstructive azoospermia involves impaired or limited sperm production within the testes. While more complex, small areas of sperm production may still exist, making retrieval possible with advanced techniques.
Sperm retrieval with TESE
A common first option for severe male-factor infertility is testicular sperm extraction (TESE), which involves removing small samples of testicular tissue to locate sperm. A reproductive specialist or urologist typically performs the procedure after assessing the potential location of viable sperm cells. TESE is useful in obstructive and non-obstructive cases. Micro TESE is a more advanced procedure that uses a high-powered microscope to more easily locate and extract possible sperm cells. This process increases the chances of locating sperm while minimizing tissue damage. Once the urologist removes the tissue, further tests confirm and extract sperm cells for assisted reproduction.
PESA and MESA
Sometimes, sperm extraction for severe male-factor infertility is possible without removing testicular tissue. If sperm is present in the epididymis, percutaneous epididymal sperm aspiration (PESA) is an effective option. PESA uses a small needle to extract sperm directly from the epididymis. Microsurgical epididymal sperm aspiration (MESA) is another technique that retrieves sperm from the epididymis using microsurgical tools. With MESA, urologists extract higher sperm counts. The best sperm retrieval method depends on the patient’s diagnosis and reproductive goals.
Combining retrieval with ART
Obtaining a sample with viable sperm is part of a multi-step process to achieve a pregnancy. The samples, whether from testicular tissue or the epididymis, are assessed to find the healthiest sperm possible. Success rates depend on the underlying cause of infertility, the technique used, and the expertise of the surgical team. In obstructive azoospermia, success rates are generally higher as there is a known presence of sperm cells. In non-obstructive azoospermia cases, success rates are more unpredictable as there could be multiple underlying causes. Healthy sperm samples are prepared for intracytoplasmic sperm injection (ICSI) instead of traditional IVF. With ICSI, a single sperm is injected directly into an egg to create an embryo. Only a small sample of viable sperm is needed to create embryos for implantation. Sperm can also be cryopreserved for future ICSI cycles.

Surgical sperm retrieval methods are a powerful tool to acquire sperm in cases of severe male-factor infertility. Techniques like TESE, micro TESE, PESA, and MESA have expanded possibilities for men or couples who once had no hope of biological family planning. With a viable sperm sample, advanced forms of ART can help couples increase pregnancy outcomes. While effective, these procedures and ICSI are not guaranteed. The fertility team will discuss possible risks, success rates, and further options, including donor sperm. These procedures can also be emotionally taxing on all parties involved, so open communication is key. As surgical retrieval techniques become more precise, men with even the most severe cases of infertility can achieve the dream of starting or growing a family.
